Use this form to submit your Scholarship Entry Application or you may submit the Scholarship Application Entry Form by mail.

This is for all full-time college students attending college Fall 2010.

Scholarship award winner(s) must provide proof of admittance.

Scholarship awards will be remitted directly to the school or reimbursed to you if tuition/books already paid.

Depending on the amount of donations received for the Scholarship Fund, there could be more than one scholarship awarded.

There will be a drawing at the banquet to determine who receives the scholarship(s), so you must be present.
