Donations Form (reply by April 30, 2010 to be listed as a Donor) This form is for anyone wishing to donate money toward additional expenses of the reunion. This is in addition to the cost of registration. All donors will be recognized. If you chose not to be recognized, please check anonymous on the form. Monetary amounts will not be listed. Evaluation Form (reply by July 10, 2010) Upon completion of the reunion please turn this form in (preferrably at the reunion). In Loving Memory of Form (Reply by April 30, 2010 to be listed) Complete this form if you'd like to donate in the memory of a loved one and have their name listed in the Family Book. Monetary amounts will not be listed. Money will be used toward general expenses or you may designate where you'd like it to be contributed RSVP Survey Form (reply by February 15, 2009) This form is a survey only. Please fill out and mail back or you can go on the website and fill in the survey under RSVP Survey. Please respond by reply date so we can get an idea of how many people will be attending and plan accordingly. If you complete this form on the website do not mail this form in. Registration Form You may fill in this Registration Form or go to Registration page and enter your information. Please note the dead- line dates to send in your money. Scholarship Application Form (reply by June 15, 2010) This form is for family members that will be attending college in the Fall 2010. From the donations we receive for the Scholarship Fund, we will be able to determine how many scholarships will be awarded and the monetary value of each. Please pass this form on to a future college student. T-Shirt Order Form (reply by May 31, 2010) Use this form to place your T-Shirt order. Don't forget to specify the color. Talent Show Form (reply by June 1, 2010) There will be a talent show if we have enough participants. Please make sure your performances are appropriate and in good taste (no vulgar language please). There will be a talent show winner. |
FYI - WEBSITE FEES:
To maintain the website there are yearly fees that have to be paid. The first 2 years I paid the fees of $14.95 per month. In 2007 the 2008 Reunion committee paid for a year. It is now due again and I am hoping to be able to pay for three (3) or five (5) years at a time through donations if possible.
